Dear Parents/Guardians,

Attached please find a Sacred Heart School Health Registration Form, an Immunization Certificate, and a Physical Exam Form.  It is a requirement that all new students have each of these on file in their registration folder before being considered for admission. The application will not be complete until all required information is received.
