 Financial Information Registration - School Year 2008 - 2009

INFORMATION NEEDED:

  • Completed Registration form
  • Sacred Heart School Health Registration form
  • A copy of Immunization records
  • A copy of current physical form
  • TB Assessment - Documented by Physician if Low Risk and does not need PPD Test or provide PPD Test Results
  • Lead Test - Kindergarten students only
  • Birth Certificate
  • Baptismal record if the child is Catholic and not baptized at Sacred Heart Parish, North Quincy
  • A Copy of all Academic Records, including the latest report card/progress report
  • A non-refundable registration fee of $100.00/child

     Transfer students entering Grades 3 - 8 must also provide the following:  
    • A copy of all Standardized Test results
    • Dates of First Penance and First Communion
    • A copy of the Student's Office Card or Permanent Record
