Dear Parents/Guardians,

A complete physical examination, current within the year, is mandatory for all students prior to the first day of school in September. This information must be on file in the student health records and a report is filed with the Massachusetts Department of Public Health. No child will be permitted to attend school without providing all necessary medical documentation.

Here are links to .pdf files for the Sacred Heart School Health Registration Form and Food Allergy Action Plan.  Please complete, sign, and return these forms, along with medical documentation, to the school office.

If your child has any special medical needs or concerns, please inform us so that we may help in accommodating them.
