Registration 2009 - 2010

Pre - Kindergarten, Kindergarten, and Grade 1
 Begins Sunday, January 25, 2009
Tours Monday through Thursday from 9:00-11:00 a.m.
 
Age requirement for school year 2009-2010:
Pre - Kindergarten - 4 years old by August 31, 2009
Kindergarten - 5 years old by August 31, 2009
Grade One - 6 years old by August 31, 2009

  • Completed Registration form (Click for .pdf file: Pre-K and K or Grades 1-8)
  • Sacred Heart School Health Registration form
  • A copy of Immunization records
  • A copy of current physical form
  • TB Assessment - Documented by Physician if Low Risk and does not need PPD Test or PPD Test Results
  • Lead Test - Kindergarten students only
  • Birth Certificate
  • Baptismal record if the child is Catholic and not baptized at Sacred Heart Parish, North Quincy
  • A Copy of all Academic Records, including the latest report card/progress report
  • A non-refundable registration fee of $100.00/child                                                                                          
Transfer students entering Grades 3 - 8 must also provide the following:
  • A copy of all Standardized Test results
  • Dates of First Penance and First Communion
  • A copy of the Student's Office Card or Permanent Record
Registration for new students begins January 25, 2009. Please call the school office for a registration packet. Prospective students and families are encouraged to visit our school. Tours are available Monday through Thursday from 9:00 a.m. through 11:00 a.m.
